Home Social Services Home Visitation

When a patient needs medical care but hospitalization is not required or when cultural expectations limit hospital or clinic access, care can be made available by Oasis Hospital nursing staff. Caring for a patient in their own home can have many benefits. It takes away the need for them to travel to the hospital or clinic. Also, working out practical ways to provide care is best done where the care will be given.

How do I receive health care visits at home?
Doctors and nurses refer patients of Oasis Hospital to our Home Visitation department. This usually happens when a patient is in the hospital or when they visit a clinic. The decision to make this referral is dependent on the specific needs of the individual patient. Many first-time mothers are referred to this service with their babies and there are a wide variety of other patients who are referred.

 

If you want to be referred by a doctor or nurse you are welcome to request a referral.
What should I expect?
Once a referral has been made you will be contacted by a member of Oasis Hospital staff. This initial contact will be done to collect information about the referral as well as to obtain contact information and directions to your house. At this time the staff person will also discuss with you what date and times will work best for you and your family to receive a visit.

 

A day or two before the planned visit a staff member will phone to confirm the visit date and time. When this call is made the details of the visit will be confirmed. If there is no response to this call, for several days the referral will be put aside and not followed.

Who will be visiting me?
A nurse and an Oasis Hospital trained volunteer will make the home care visit and will bring all the needed equipment and information with them.

 

Is the Home Visitation Service confidential?
All visits made by Oasis Hospital nurses and volunteers are documented and kept as part of the patient's file. These visits are part of Oasis Hospital care and so fall under the same regulations of confidentiality as all care provided at Oasis Hospital.

 

If you have missed a call from Oasis Hospital home health care team or if your plans change and you need to change a scheduled appointment with our home care team please call them during the day at 050-3150-271 Saturday thru Thursday. They are not available on Fridays.
